POSITION DESCRIPTION & OVERVIEW
The Housing Programs Department serves households who are experiencing or at risk of homelessness in Hennepin County. Our programs include Prevention, Coordinated Entry Assessments, Fast Track, Rapid Rehousing, and Long-term Supportive Housing for households with children and adults without children. By offering programs of various intensity and length, we hope to be able to provide the level of service each household needs to be successful. The goal in all of our housing programs is to end homelessness by assisting households in stabilizing their housing and achieving self-sufficiency.
The Prevention Case Manager is primarily responsible for assisting families and singles who are at imminent risk of losing their housing and have histories of homelessness. The Prevention Case Manager will assess each household's strengths and needs, assist the household in gathering appropriate documentation to submit an application to RentHelp. The Prevention Case Manager will monitor spending and services to ensure that program requirements are fulfilled and the program meets its budgetary expectations.
PRIMARY DUTIES AND EXPECTATIONS
Assist participants with stabilizing their housing (i.e., intake, assessment).
Maintain regular contact with landlords, negotiate payment arrangements, and mediate landlord-tenant issues.
Provide financial assistance for families in compliance with program requirements and budget.
Assist with program reporting, ensuring that program is on track to meet contractual goals.
Coordinate with Early Intervention Program Manager and Director of Housing Programs to ensure compliance with funding expectations.
Establish and maintain active working relationships with community partners and other providers.
Connect families to mainstream resources that will continue after the termination of our services.
Educate clients about tenant responsibilities and rights, financial management, and affordable housing opportunities.
Complete accurate case notes, data collection, and other documentation for assigned clients in a timely manner.
Attend grantee meetings by funder.
Other duties as assigned.
Position consists of approximately 50% direct client contact, and 50% indirect client-related work (i.e. documentation, communicating with collateral partners, preparing for client meetings, attending trainings and community events).
PHYSICAL REQUIREMENTS FOR THIS POSITION
Sedentary work - Sitting most of the time with occasional walking. Exerting up to 10 pounds of force occasionally.
Must be able to attend occasional home visits and apartment showings with clients, including units that may only be accessible by climbing stairs.
REQUIRED QUALIFICATIONS
Have skills and knowledge acquired through at least one of the following:
A course of study in a health or human services related field leading to a Bachelor of Arts, Bachelor of Science, or associate's degree;
One year of experience with the target population served;
Experience as a certified peer specialist according to Minnesota Statutes, section 256B.0615; or
Meet the requirements for unlicensed personnel under Minnesota Statutes, sections 144A.43 to 144A.483.
Hold a current driver's license appropriate to the vehicle used if transporting recipients of Housing Support.
Completion of vulnerable adult mandated reporter training (or child maltreatment mandated reporter training where applicable).
Completion of orientation training when offered by the Department.
Background Study Requirements
The following individuals are subject to background studies according to Minnesota Statutes, section 245C:
Controlling individuals as defined in section 245A.02; and
Managerial officials as defined in section 245A.02; and
All employees and volunteers of the establishment who have direct contact* with Housing Support recipients, or have unsupervised access to Housing Support recipients, their personal property, or their private data.
A Housing Support recipient cannot provide direct contact* services to other Housing Support recipients.

Experience that gives applicants the required skills, abilities, and perspectives listed above are commonly gained through one or more of the following:
Personal experience with homelessness;
1-year documented experience working, interning, or volunteering with people experiencing homelessness;
1-year documented experience supporting singles and families as a Case Manager
An Associates, Bachelor's, or Master's degree in a relevant field such as Social Work (with accompanying licensure), Human Services, or Family Studies.

This position requires use of your own personal vehicle for Agency business and will be required to maintain liability insurance coverage of at least $100,000 per person and $300,000 per occurrence. In addition to criminal background studies, applicants must have a valid Driver's License and pass a driving record background check.
PREFERRED QUALIFICATIONS
Two years' experience working with families and/or single adults experiencing homelessness.
Lived experience of homelessness.
Licensed Social Worker.
Fluency in languages used by our clients (Spanish, Amharic, Oromo, Somali, ASL)
REFERRAL BONUS PROGRAM
