The RN Care Coordinator will work with a multidisciplinary team to manage high-risk patients, develop care goals, and provide disease management coordination. They will also serve as a patient advocate, facilitate transitions of care, and conduct comprehensive clinical assessments to improve patient outcomes.
Candidates must be graduates of an accredited school of nursing with a current RN license and three to five years of nursing experience. A BLS certification from the American Heart Association is required, while a BSN and specialty certification are preferred.

Cleveland Clinic Care Coordinators have been very successful in helping patients manage their own care. In this role, your hard work, dedication and commitment can lead to a decrease in Emergency Department visits, observation status, inpatient stays and hospital readmission in care coordinated patients.
A caregiver in this position works a hybrid schedule from 8:00am to 4:30pm or 8:30am to 5:00pm, with the availability to work from home 2 days per week. This role will be stationed at Crile Building at CC Main Campus.
