Position summary: Reporting directly to the Director of Health & Youth Development, the Health Core Fellowship Program Coordinator II plans and facilitates the full cycle of the Health Core Fellowship Program — from curriculum design and youth facilitation to the external partnerships (City College San Francisco, hospitals, clinics, etc.) that make certifications, mentorship, and paid internships possible. This role blends direct youth-facing program delivery with the relationship management and outcomes tracking needed to move a cohort through a full year and into CHW employment or continued training. The Coordinator is accountable not just running engaging sessions, but for building the CHW pathway, partnerships, retention systems, and mentor program infrastructure that determine whether the fellows actually complete the program.
Duties & Responsibilities:

Program design & implementation:
Design & facilitate a phased program structure (foundational coursework, applied coursework, and supervised CHW internship) aligned to health care professional core competencies
Coordinate skill and certification trainings (BLS/CPR, First Aid, HIPAA) that align with common health workforce needs
Coordinate site visits to local health agencies and clinics
Manage fellows’ enrollment in and completion of City College of San Francisco’s Community Health Worker certificate program, ensuring coursework functions as the credentialing backbone of the fellowship
Partnership & internship development
Build and maintain a year-round pipeline of paid CHW internship placements with the SF Dept. of Public Health, UCSF, SF General Hospital, and other local health agencies and clinics, including proactive outreach to new partners each year
Serve as the primary point of contact for community and institutional partners, including CCSF, fiscal sponsors, and health agency stakeholders
Represent 3rd Street and the Health & Youth Development team at external events and community meetings build and sustain the CHW partner network
Mentorship program
Recruit, screen, and retain mentors with clear, bounded expectations and milestones
Plan and facilitate engaging mentor-mentee events, including monthly dinners
Monitor mentor-mentee engagement and troubleshoot mismatches or disengagement
Fellow recruitment, retention, & support
Recruit program participants through targeted outreach and structured interview process
Design and monitor a retention and accountability system for fellows, including attendance tracking and early-warning check-ins for disengagement
Data & admin
Collect program data and maintain excellent recordkeeping
Track and report program outcomes — CHW certificate completion rates, internship placement, and post-program CHW employment or enrollment in further training
Support events hosted or attended by the organization and the Health & Youth Development team through attendance, outreach, planning, and participation
Maintain high professional standards of performance, demeanor, communication, and appearance at all times
Perform tasks and responsibilities in a complete and timely manner
Support with other related responsibilities as needed
About you:
Skills, Knowledge, and Abilities (SKA):
Ability to engage, inspire, and educate teenage and young adult audiences
Ability to facilitate workshops with large groups
Knowledge of the Community Health Worker (CHW) model, scope of practice, and core competencies
Demonstrated ability to build and sustain institutional partnerships (e.g. with healthcare systems, higher education, or government agencies)
Project management skills — able to run multiple concurrent workstreams (curriculum, mentorship, internships)
Good communication skills, both verbal and written
Strong interpersonal skills, including empathy, humility, and conflict resolution
Experienced in Microsoft (Word, Excel, PowerPoint) and/or Google Suite (Doc, Sheets, Slides, Drive); experience in creating designs on Canva is a plus
Education & Experience:
Bachelor’s degree in public health, biology, health sciences, health education, behavioral sciences or related field
3–5 years of combined experience across: (a) direct work with transitional age youth of diverse backgrounds, and (b) program coordination or partnership/stakeholder management, ideally in workforce development, healthcare, or CHW-related fields
Experience managing partnerships or contracts with external institutions (healthcare systems, higher education, government agencies) strongly preferred
Experience in community organizing or community development is a plus
Physical demands:
Ability to lift and carry 25 lbs
Valid driver’s license and access to reliable transportation
Availability (if appropriate): for example, must be available to work weekends

Compensation and benefits:
Salary is within a range of $75 – $80K, depending on experience, qualifications, and internal equity. The upper end of the range reflects growth potential over time in the role.
