The Intake Coordinator manages the initial contact for new hospice referrals and coordinates the enrollment process for new patients. They are responsible for gathering medical documentation and communicating with RN Case Managers and medical professionals.
Candidates must have previous experience in hospice intake and a valid driver's license. Proficiency with Homecare Homebase software is highly preferred, along with the ability to pass background and drug screenings.
