Focused Post-Acute Care Partners (FPACP) is more than just a company of nursing homes. Our focus is partnering with our skilled team members, residents and their family members to determine the most appropriate care based on the residents’ ever- changing needs. Skilled nursing and long-term care remain central to our care mission, however, we also provide inpatient and outpatient short-term rehabilitation, post-acute short-term rehabilitation and post-operative recovery. We partner with highly trained and skilled professionals to consistently achieve the clinical goals of each resident.

A number of our communites have developed an expertise in delivering Alzheimer's and dementia care and our staff maintain training and education to stay updated on the latest care strategies.
