The Resource Nurse is responsible for conducting pre-assessments and admissions for hospice patients in homes or healthcare facilities. They collaborate with intake teams and physicians to ensure the appropriateness of referrals and provide emotional support and education to families.
Candidates must be a licensed Registered Nurse with at least one year of experience in a medical-surgical or acute care setting. A bachelor's degree in nursing is preferred, along with current CPR certification and a valid driver's license.
Hospice of Florida is currently hiring a Full time Compassionate Resource Nurse (RN) to provide patient care in and around the Jacksonville Area!
Our Resource Nurses (RN) are responsible for assuring that the Hospice programs are effectively represented to patients, families, and others who have an interest in hospice through pre-assessments, information sharing and/or admissions in a Patient's home or in a health care facility. This position works with the intake team, referring physician, and other referral sources to promote the hospice philosophy of care.
Schedule: Monday - Friday, 11 am - 8 pm
