The Outreach Worker provides assessment, service planning, and resource navigation to support older adults and their families in Calgary. The role involves managing a caseload, collaborating with community partners, and conducting home visits to strengthen social inclusion.
A Bachelor's degree in social work or a related field and 2-3 years of experience in community outreach or social services are required. Candidates must possess a valid driver's license, a reliable vehicle, and experience working with older adults.

The Outreach Worker supports older adults and their families across Calgary to access resources, strengthen social inclusion, and participate more fully in their communities. Working collaboratively with participants, families, Carya colleagues, and The Way In Network partners, this role provides assessment, service planning, resource navigation, advocacy, and community-based support.
