Position Overview
SCOPE OF ROLE: Under the supervision of the Program Director, the Assistant Program Director will provide supervision of case management services to ensure quality care for adults that have histories of homelessness and are diagnosed with severe mental illness. In the absence of the Program Director the Assistant Program Director will supervises all program staff in conjunction with the Regional Director.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Ensure program complies with all Federal, State and Local laws and regulations as they apply to supportive housing programs as well as all HRA/DSS regulations/requirements
Ensure housing and performance outcomes are met on a monthly/quarterly/annual basis
Assist the Program Director with the administration of the program, overseeing all aspects of the program’s operations - service delivery, compliance to contracts, targets (case management, employment, medical and chart compliance, recreational activities, security, maintenance, food service if applicable, etc.)
Provide and ensure delivery of quality services to all residents through direct supervision of all program staff regarding case management
Ensure that all documentation (including, but not limited to, progress notes, service plans, assessments, health forms, reports, etc.) is completed by direct service staff in a timely manner and within established deadlines as per agency and funding sources standards.; provide coaching on policies and procedures and clinical practice.
Development and implementation of individual client service plans in conjunction with clinical staff, counseling team and client
Responsible for ensuring that contacts with clients reflect goals of service plans and assisting wellness coaches to develop intervention goals and activities which support the service plan
Coordinate annual functional assessment review, psychosocial review, chart audits for each client and submitting report to Program Director.
Responsible for maintaining contact with other service agencies and insuring client placement for treatment and program activities (may include volunteer job placement)
Accountable for coordinating orientation and program entry for all new residents with the intake coordinator; participates in discharge planning
Assist Program Director in serving as liaison to other community services; attend meetings and develop new linkages; provides emergency and relief coverage as necessary
Prepares various reports for funders, Regional Director, and AVP of Behavioral Health Housing Services, including any issues, and suggestions for changes in policies or program adjustments
Conduct and document supervisory meetings with direct reporting staff, promote staff development; and evaluate their work performance (i.e. identify training needs)
Ensure that all documentation pertaining to client and program activities is accurately completed, handled confidentially, kept in the proper location, and submitted promptly.
Ensure all incident reporting and follow up is done thoroughly and in a timely manner

REQUIREMENTS:
REQUIRED EDUCATION AND EXPERIENCE
Have an understanding, appreciation, and commitment to the philosophy and mission of Services for the UnderServed (S:US), with expertise in harm reduction and recovery-oriented services
Master’s degree in social work or related field
Three to five years human services experience
Two years of supervisory experience
PREFERRED QUALIFICATIONS & SKILLS
Bilingual in Spanish , preferred
Familiarity with Federal, State and Local laws and regulations as they apply to supportive housing programs and HRA/DSS regulation/requirements for NY 15/15 Supportive Housing
