JOB SUMMARY The primary role of the Behavioral Health Care Coordinator is to provide care management and administrative support to help patients access behavioral health, substance use, and social services tailored to their individual needs. This position supports the Behavioral Health (BH) team by facilitating service delivery, advocacy, and education for clients and the community. The Behavioral Health Coordinator also assists patients in connecting with additional resources, such as psychiatric services, referrals for a higher level of mental health care, and other community-based support services.

ESSENTIAL DUTIES & RESPONSIBILITIES
To perform the job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required:
Clerical and Administrative Tasks
Assist BH providers with patient paperwork, faxes, and correspondence (e.g., patient progress letters, treatment-specific forms for consultation, scanning BH documents into patient charts, no-show letters, etc.).
Ensure all patient communication is documented completely and scanned timely into the electronic health record (EHR), including obtaining signed consent forms for services
Follows the no-show policy and procedure and ensures all correspondences are documented in a telephone encounter or note in eClinicalWorks. Documentation will be performed in a timely and accurate manner in accordance with program policies and professional standards of care.
Coordinate phone triage for incoming calls to the Behavioral Health department.
Serve as a liaison for the medical and dental care teams. Based on the needs of the patient, will provide an overview of BH services and schedule an appointment or a same-day warm handoff (WHO).
Manage BH appointments with a focus on keeping the schedule full (goal = 6 completed visits per day per provider), which may include managing the BH waitlist and determining whether to schedule a same-day appointment.
Support the BH provider in matching patients appropriately with an individual therapist with regards to level of urgency, patient preference, language preference, and availability of clinician.
Generate Behavioral Health (BH) reports as needed, including CCALAC BH audit tool reports, provider visit summaries, and other relevant documentation.
Verify patients’ insurance eligibility
Under the guidance and supervision of the Behavioral Health Director, serve as a resource and train new and current non-behavioral HarborCHC staff on Behavioral Health protocols (e.g., scheduling, work flow, managing mental health crises, etc.).
Maintain patient confidence and protect operations by keeping patient health information confidential as determined by HIPAA regulations.
Perform other duties as assigned to support HarborCHC Mission and Values.
Behavioral Health Case Management
Coordinate referrals for continuity of patient care with external healthcare organizations and community resources; follow up with patients and organizations, as appropriate, to document use and success of referral.
Administer patient screenings (e.g., PHQ-9, GAD-7)
Use motivational interviewing techniques or de-escalation techniques with patients as needed in the interim of connecting the patient with a BH provider
Participate in outreach activities to educate and inform the community of integrated behavioral health and other available services at HarborCHC
Maintain and update the behavioral health community resources list, including information for the point of contact of each community partner.
Serve as a liaison with the Patient Care Coordinators once the BH Providers identify gaps in the social determinants of health (housing, job resources, food insecurity, etc.).
Attend trainings, conferences, and meetings as required.
Perform other duties as assigned to support HarborCHC’s Mission and Values.
QUALIFICATIONS
Required education, experience, and/or training:
Bachelor’s degree in social work or related field of study and 1 year of experience working in a behavioral health setting.
Ability to work with people of diverse cultural, educational, social, and economic backgrounds.
Ability to organize and prioritize work tasks.
Knowledge of HIPPA compliance for patient/agency records according to established professional social standards.
Ability to work as part of a multidisciplinary team.
Knowledge of community mental health and social services.
Computer and internet proficiency.
Coordinate long-term therapy referrals for patients with chronic/severe mental illness.
Excellent oral and written communication skills.
Excellent organizational skills.
Experience utilizing an Electronic Medical Records (EMR) system is highly desirable.
Bilingual English/Spanish required.

Hours: HarborCHC is open Monday-Thursday 7:00am-7:00pm, Friday 8:00am-5:00pm, and Saturday 8:00am-5:00pm with occasional overtime or other shifts based on clinic need. This is a non-exempt position, and any overtime must be approved by the supervisor in advance.
