Mental Health Resource Center is a not-for-profit Florida corporation that provides a wide range of mental health and behavioral health care services to the community such as 24-hour emergency services, inpatient psychiatric services for children, adolescents, and adults as well as outpatient services such as medication management, case management, and counseling.
Job Summary
The System of Care (SOC) Navigator serves as the central referral hub for MHRC programs and community resources. This position coordinates internal and external referrals, facilitates warm handoffs, addresses barriers to care, and promotes timely access to integrated, person-centered services.
A primary focus is ensuring uninsured and underinsured individuals are connected to appropriate services within seven (7) days of discharge, including psychiatric and medication follow-up when needed. The SOC Navigator engages individuals prior to discharge, provides education regarding MHRC’s Comprehensive Services Center (CSC) Central Receiving System, facilitates intake and service connections, and follows up to confirm successful linkage.
The SOC Navigator also identifies individuals at risk for repeated acute-care utilization and provides outreach, referral coordination, and continuity-of-care support to help prevent unnecessary crisis episodes, Baker Act admissions, and re-hospitalizations through least-restrictive interventions.
Responsibilities
Care Coordination & Referral Management
Serves as the primary point of contact for MHRC and community referrals.
Receives, triages, tracks, and follows up on referrals to ensure timely and appropriate linkage to care.
Facilitates warm handoffs and coordinates with behavioral health, medical, social service, housing, and community providers.
Maintains knowledge of MHRC programs, intake pathways, and community resources.
Discharge & Hospitalization Prevention
Supports uninsured and underinsured individuals in accessing services within seven (7) days of discharge.
Links individuals requiring psychiatric or medication services to appropriate care within seven (7) days and follows escalation procedures when timely appointments are unavailable.
Participates in treatment and discharge planning meetings and identifies individuals needing bridge appointments or enhanced follow-up.
Conducts outreach to high utilizers of crisis stabilization, inpatient, and detoxification services.
Provides post-discharge follow-up, rescheduling assistance, and barrier resolution.
Advocacy & Community Engagement
Identifies and addresses Social Determinants of Health (SDOH) barriers, including transportation, housing, identification, and other obstacles to care.
Advocates for individuals and supports their rights in accordance with applicable laws and regulations, including the Baker Act.
Builds rapport with individuals and their chosen supports and encourages involvement when appropriate.
Serves as a liaison to community partners and promotes awareness of MHRC services and referral pathways.
Documentation & Compliance
Maintains accurate and timely documentation of contacts, referrals, outreach, follow-up, assessments, and required consents/releases.
Completes required reports and monitors referral outcomes to identify service gaps and improvement opportunities.
Participates in quality improvement activities and complies with organizational and regulatory requirements.
Recognizes and responds appropriately to safety risks, trauma, abuse, neglect, abandonment, emergencies, and other emergent concerns.
Exercises sound independent judgment in performing job duties.
Documentation Tool (Eleos Health)
Reduces time spent on documentation, allowing more focus on client interaction and care
Transforms brief notes or session conversations into complete, clinically appropriate documentation
Supports accurate, compliant documentation while allowing staff to review and finalize notes
Provides helpful insights from sessions to support care planning and continuity
Accessible on desktop or mobile for flexibility across different work settings
Qualifications
Bachelor’s degree in Social Work or a related Human Services field from an accredited college or university required. A related Human Services field must include at least 30 hours of coursework addressing human behavior and development.
One (1) year of experience in human services, behavioral health, or a related field required.
Experience working with adults with mental illness preferred.
Strong communication, interpersonal, organizational, and problem-solving skills required.
Ability to work independently, prioritize multiple referrals, and respond appropriately to urgent situations.
Proficiency in Microsoft Office, Outlook, and Internet-based resources required.
Proficiency in the RBHS/MHRC Electronic Health Record (EHR) and Patient Information System must be demonstrated within three (3) months of employment.
Position Details This is a Full Time Days position: Monday through Friday, 8:00am to 4:30pm.
