The LPN will collaborate with RNs and physicians to provide direct patient care, including monitoring vital signs and performing specialized procedures in a home setting. They are also responsible for teaching self-care techniques to patients and caregivers while maintaining accurate clinical documentation.
Candidates must hold a current LPN license and have at least one year of clinical experience. A valid driver's license, reliable transportation, and current CPR certification are also required.
Verify plan of care with the RN prior to each visit.
