The Diversion Float Case Manager works as part of all three Diversion teams to provide outpatient behavioral health services and community support to our client population. This position is responsible for providing services and care coordination based on client needs, program goals and individual documented plans, progress notes, and collaborative reviews for the assigned caseload, as well as support to all clients served as part of the care team.

Monday-Friday, 9:00am-4:00pm (35 hours/week)
Serve as a Float Case Manager covering program needs when short staffed including advocating for and coordinating client needs and services; ensuring Service Plans, Crisis Plans, and other documentation are completed on time and kept up-to-date, as applicable; and providing consistent client contacts, skills training, and care coordination to work towards goals of the client, program, and contract requirements.
Coordinate with relevant internal and external departments on resident referrals, ensure timely intake into program, and review individualized plans.
OCRP staff identify barriers to competency and complete weekly reporting to the courts.
FHARPS staff collaborate with clients, members of our legal systems, and multidisciplinary teams across the behavioral health spectrum of care to determine housing needs, create plans appropriate to this need, and engage clients in accessing resources to reach the goal of improving housing status.
FPATH staff collaborate with clients, members of our legal systems, and multidisciplinary teams across the behavioral health spectrum of care to determine goals related to basic needs, attaining benefits, and establishing behavioral health and physical health treatment.
Collaborate with care team to ensure clients access needed services, support, case management, and appointments, including transportation to external providers.
Case Managers with OCRP must be familiar with Breaking Barriers curriculum and facilitating groups, coordinating interpreter services, and administering urinalyses.
Outreach clients in their homes or where they are staying in the community to assess living situation and stability and provide treatment interventions when necessary.
Ensure all interactions represent the five principles of trauma-informed care, including safety, choice, trustworthiness, collaboration, and empowerment.
Coordinate access to medications for all clients through trained staff ensuring the seven rights of medication use are followed: right person, right medication, right time, right dose, right route, right reason (including refusal), and right documentation according to agency provided medication training.
Complete daily documentation in electronic medical record on client interactions for billable and non-billable activity, medications dispensed where applicable and assigned, and other required documentation, ensuring an interaction and subsequent documentation are completed for all assigned clients on a consistent basis, as determined by program requirements.
Practice good financial stewardship of agency and contract resources, including how time is spent, supplies are used, properties are being maintained, etc.
Monitor spending of Petty Cash and other agency money to ensure accountability and appropriate use.
Meet identified key performance indicators and metrics for the position as identified for the program, division, and agency.
Participate in discharge planning and written summaries of services as needed for client referral and releases of information.
Provide immediate crisis intervention and resolution as needed and trained.
Participate in the staff rotation of the drop-in center to serve clients, as assigned.
Always maintain client and staff confidentiality.
The Diversion Float Case Manager works with care team to ensure high standards of care are provided within the program, including but not limited to:
Ensuring appropriate levels of service, skills training, and assistance are provided to clients in managing mental health symptoms and developing skills and insight to manage symptoms.
Participating in staff rotation of Drop-In Center to ensure adequate staffing of the space and supporting the needs of clients.
Partnering as a team to deliver current programming and fill vacant position duties.Working collaboratively with larger outpatient team of providers to support clients in actively working towards treatment goals.
Coordinating care, such as scheduling, attending, and/or arranging transportation for appointments and other case management needs.
Maintaining and documenting in client charts and program trackers following agency, county, state, and federal guidelines.
Providing and receiving input as part of resident’s care team on service planning and progress, and collaborating with other providers such as primary care providers, psychiatric prescribers, other housing staff for managing housing, etc.
Facilitating regular group and individual activities, encounters, and specialized client events, such as outings and holiday gatherings.
Performing other duties as assigned.
QualificationsQUALIFICATIONS
Bachelor’s degree in social science or other related field, may consider candidates who have 2+ years of relevant experience serving adults with severe and persistent mental illness and/or in diversion services in lieu of degree. For the FHARPS program, credentialing as a Certified Peer Counselor is acceptable and preferred.
Apply for Agency Affiliated Counselor credential with Washington Department of Health within 30 days of hire, if not otherwise licensed, and obtain and maintain registration and/or licensure. For the FHARPS program, credentialing as a Certified Peer Counselor is acceptable and preferred.
Successfully pass criminal background check before hire date.
Obtain/maintain valid First Aid/CPR card or certificate within 30 days of hire.

Able to work independently and within a team.
