The Massachusetts Department of Mental Health (DMH) Metro Boston Fuller Site is seeking a Human Services Coordinator I (HSC) with strong attributes in the Human Services field to work in the Greater Boston Area. The successful candidate for this position will work with adults who have been determined to be eligible for DMH services. The core function of this position is to help individuals maintain their stability in the community, provide resources, crisis stabilization and assist with assessment of their mental health needs and psychiatric services.
Please Note Schedule: Monday through Friday, 8:00am to 4:30pm, with Saturday and Sunday off.
Duties and Responsibilities (these duties are a general summary and not all inclusive):
Implements creative engagement strategies, including regularly scheduled face-to-face contact, in accordance with the needs of the individual, family, and community providers.
Promotes sustained involvement and collaboration to help the person served achieve their goals and maintain community tenure. Carries a case management caseload and will help clients locate and maintain; housing, financial assistance, medical, mental health and dental services, vocational and educational resources, socialization opportunities and transportation, to support clients successfully transition to and placement in the community.
Develop ISP for clients in caseload to ensure coordination and monitoring of all aspects of treatment for each client.
If necessary, provide outreach and crisis intervention to clients to assess mental health needs and ensure proper psychiatric treatment.
Maintains and initiates extensive liaison work with community agencies including mental health services, court, welfare, and social security, to maximize the quality of life for clients.
Collects standardized client related data for research, evaluation, or administrative purposes.
Maintains clients' records with up-to-date client information, progress notes and ISP, to ensure coordination and adequate treatment services.
Visits clients in their homes or usual place of residence and monitors the safety of the environment.
Facilitates referral of clients to programs best suited to their service’s needs. Coordinates transitions to various community programs.
Safeguards human rights of clients through familiarization with providers and domestic environment, support and advocacy and reporting of human rights.
Performs related duties as assigned.
Required Qualifications:
Knowledge of psychosocial problems of those with mental/emotional disorders. · Awareness of principles, practices, and techniques of counseling.
Knowledge of the types of programs and services available to the mentally ill population. · Knowledge of principles and practices of crisis intervention.
Capacity to evaluate and maintain accurate records.
Ability to make decisions act quickly and maintain a calm manner in stressful and/or emergency situations.
Ability to write concisely to express thoughts clearly and to develop ideas in logical sequence.
Preferred Qualifications:
Experience working with individuals with serious, persistent mental illness, substance abuse and/or who have experienced homelessness.
Given the served population, bilingual or multilingual fluency.

Based on assignment as Qualified Intellectual Disabled Professionals, within the Department of Developmental Services, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or other a related field is required.
Salary placement is determined by a combination of factors, including the candidate’s years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division’s Hiring Guidelines. For all bargaining unit positions (non-management), compensation is subject to the salary provisions outlined in the applicable collective bargaining agreement and will apply to placement within the appropriate salary range.
