POSITION SUMMARY
The Comprehensive Perinatal Health Worker (CPHW) works to ensure members receive access to high quality health care and that their care is coordinated. The CPHW will adhere to the Comprehensive Perinatal Services Program (CPSP) model of care, and provide administrative support through processing referrals, managing medical records, and scheduling appointments.
The Native American Health Center is an accredited institution that adheres to the standards of excellence set forth by the Accreditation Association of Ambulatory Health Care (AAAHC) and the Commission of Dental Accreditation (CODA).
DUTIES & RESPONSIBILITIES
Direct Service Responsibilities
Provide new member orientations for perinatal members and conduct ongoing CPSP assessments based on CPSP model, protocols and workflows.
Provide care coordination in collaboration with Care Team (i.e., provider, MA, Care Coordinators). Identify levels of risk, consult with clinicians and make
appropriate referrals as needed.
In conjunction with Referral Coordinators, process perinatal referrals, and assist with scheduling external appointments for members.
Track referrals and follow up with members and specialists to ensure members attend specialist appointments. This includes scheduling outside OB ultrasound and Non-Stress test visits, Quest lab appointments, inductions and genetic testing..
Ensure each member receives, understands, and follows an Individualized Care Plan (ICP.)
Problem solve barriers to care with member and members’ Care Team, including assisting members with transportation, translation and interpretation needs.
Ensure documentation, including billing codes, are current, correct, and optimized.
Perinatal Program Administrative Responsibilities
Answer incoming calls to Perinatal department. Resolve issues, schedule appointments, redirect and or escalate as necessary.
Schedule members for orientations, assessments, eligibility and medical appointments (internal and external) and follow up on missed appointments daily
(i.e., rescheduling.)
In conjunction with Medical Records, manage perinatal member medical records. This includes ensuring all records are complete, documents are scanned into EHR timely, and that external organizations (i.e., hospitals, specialists, etc.) have copies of records as appropriate.
Conduct appointment reminder calls.
Prepare OB risk assessment forms.
Track statistics and prepare reports, and share on a regular basis. This includes maintaining patient and panel reports and rosters.
Prepare and facilitate the multi-disciplinary Case Conference. Act as the administrator for the Case Conference (i.e., meeting logistics).
Under the guidance of the Medical Assistant (MA) Supervisor, and in collaboration with medical leadership, develop and update written protocols for each CPSP service.
Participate in NAHC’s overall CPSP efforts and initiatives.
Participate in external meetings and initiatives. Ensure NAHC is wellrepresented.
Maintain and manage supplies and other program materials to ensure a constant level. This includes responsibility for the emergency birthing kit.
General Duties and Responsibilities
Member Care: Demonstrate understanding and apply working knowledge of safety policies and ensuring safe member practices.
Employee Safety: Safely performs all duties; follows required protective protocols to ensure personal safety as well the safety of others.
Must maintain compliance with ergonomic safety standards; be mindful of posture and regularly practice ergonomic stretches.
Quality Improvement: Actively participate in internal quality improvement teams and work with members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards, when assigned.
HIPAA: Keep all protected health information (PHI) confidential and abide by HIPAA policies for the release and disclosure of any PHI. Will report unauthorized use of disclosure of PHI immediately, to supervisor or HIPAA security officer.
Work well under pressure, meet multiple and often competing deadlines.
At all times demonstrate cooperative behavior with supervisors, subordinates, colleagues, clients and the community.
Other duties as assigned by Supervisor.
Scope of Practice
At all times, keep charting current. Documentation must be complete within 24 hours.
Maintain a provider template with a goal of four (4) patients seen per day, while ensuring optimal billing practices.
Follow up on missed appointments, daily.
Ensure documentation and files are audit-ready at all times.
QualificationsMINIMUM QUALIFICATIONS
Must have worked in a Perinatal program for a period of at least 12 months, or have a bachelor’s degree in Health Education.
Fluency in both English and Spanish, both written and verbal may be required per operational need.
Vaccination Level 1: Must be able to provide TB, Physical, seasonal flu and proof of Vaccination immunity for Measles, Mumps, Rubella, Varicella, TDAP &
Hepatitis B, prior to start date. If vaccination records are more than 10 years old, titers are required to confirm immunity.
BLS/CPR certification must be obtained prior to start date and kept current at all times.
Must be computer literate; and have familiarity with Microsoft Office programs.
Must have superior customer service skills.
Ability to multi-task and must be self-motivated.
Be able to lift, bend, carry up to 25lbs with relative ease, and to sit for long periods of time.
PREFERRED QUALIFICATIONS
Experience working in a community health setting.
Experience working with an electronic health record system
