Position Summary
The Health Home HARP Administrator is responsible for overseeing the administration, compliance, and operational performance of Health Home services for individuals enrolled in New York State's Health and Recovery Plan (HARP). The Administrator ensures adherence to Medicaid, Health Home, and HARP regulations while supporting care management staff, monitoring program outcomes, coordinating with Managed Care Organizations (MCOs), and promoting high-quality, person-centered care for individuals with significant behavioral health needs. The role serves as a liaison between internal departments, external partners, and regulatory agencies to ensure effective service delivery and program integrity. HARP programs are designed to coordinate physical health, mental health, substance use, and community-based services for individuals with complex behavioral health needs.
Key responsibilities and essential job functions include, but are not limited to:
Program Administration & Oversight
Manage daily operations of the Health Home HARP program.
Ensure compliance with New York State Department of Health (DOH), Office of Mental Health (OMH), Medicaid, and Health Home requirements.
Develop, implement, and monitor policies, procedures, and workflows related to HARP services.
Maintain program readiness for audits, reviews, and quality assurance activities.
Monitor enrollment, outreach, engagement, and retention activities.
Compliance & Quality Management
Ensure timely completion of required assessments, care plans, and documentation.
Monitor compliance with HARP, Health Home, BH HCBS, CORE, and HH+ requirements.
Conduct chart reviews and quality assurance audits.
Track program performance metrics and implement corrective action plans when necessary.
Prepare and submit required reports to leadership, Health Homes, MCOs, and regulatory agencies.
Care Management Support
Provide guidance and technical assistance to Care Managers regarding HARP eligibility, assessments, service planning, and documentation.
Support staff in utilizing person-centered, recovery-oriented, and trauma-informed practices.
Assist with complex cases and service coordination challenges.
Conduct client intakes as needed.
Ensure care plans address behavioral health, physical health, housing, employment, education, and social support needs.
Stakeholder Collaboration
Serve as the primary liaison with Health Homes, Managed Care Organizations, county agencies, and community providers.
Participate in interdisciplinary meetings and collaborative initiatives.
Build and maintain relationships with behavioral health, medical, housing, employment, and social service providers.
Coordinate responses to external inquiries and program-related concerns.
Data Management & Reporting
Monitor program utilization, outcomes, and key performance indicators.
Analyze trends and identify opportunities for process improvement.
Maintain accurate records and ensure data integrity within electronic health record and care management systems.
Prepare reports for executive leadership and funding entities.
Staff Development
Assist in onboarding and training staff on HARP and Health Home requirements.
Provide ongoing education regarding regulatory updates, best practices, and documentation standards.
Support professional development initiatives for care management staff.
Special projects and other duties assigned.
Desired skills and experience:
Bachelor’s degree in a human services, healthcare, or related field and at least three (3) years of experience in Health Home, HARP, Medicaid managed care, behavioral health, or care management programs, including one (1) year of program leadership or supervisory experience is required.
Master’s degree, professional licensure (LMSW, LCSW, LMHC, RN, CASAC), and extensive knowledge of NYS Health Home, HARP, and Behavioral Health HCBS regulations is preferred.
Demonstrated ability to communicate effectively and work cooperatively with culturally diverse persons, staff and community service providers.
Knowledge of local behavioral health services and substance abuse agencies.
Ability to multi-task in a fast-paced environment, possess good problem-solving skills, as well as excellent time management and organizational skills and the ability to remain calm in a crisis while providing crisis intervention.
Must possess computer literacy (Internet, MS Office suite, etc.) and proficiency in the use of electronic health records and ability to utilize multiple platforms.
An in-office presence of 3-4 days/week is required.
Compensation range: $48,000.00 - $58,000.00
