Provide skilled physical therapy to patients with various diagnoses and disabilities within a home setting. Manage treatment plans in accordance with physician guidelines to maximize patient independence and functional levels.
Requires a degree in Physical Therapy (Bachelor's, Master's, or DPT) and a valid Oregon Physical Therapist License. Candidates must have BLS certification and at least one year of clinical experience in home health or a healthcare setting.

Schedule: Full time, Days, Monday- Friday Day shift
Manages and provides skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician plan of treatment, taking into account individual patient special or age-related needs. Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential.
