Responsibilities
Coral Shores Behavioral Hospital is an 80-bed behavioral health facility located in Stuart, FL, and provides inpatient treatment for children, adolescents, adults, and senior adults. Our professional staff individualizes care to uniquely fit the intensity and severity of each patient case and their family's needs. Because crisis occurs at all times of the day and night, our services are provided 24 hours a day, 7 days a week.
The Step-Down Navigator at Coral Shores Behavioral Health is responsible for engaging with eligible patients to discuss the benefits of step-down services as part of their treatment continuum. This role involves communication with senior leadership and departments regarding patient eligibility, as well as data collection and reporting to support program expansion and treatment accessibility.
Responsibilities
Identifies and meets with 100% of eligible patients to discuss the benefits of appropriate programming options for patients
Ensures all aspects of the step down process are followed: identification of eligible patients, insurance verification, treatment team plan approval, etc
Assist with discharge planning for PHP/IOP patients already enrolled in the program
Maintains electronic data collection process and provides weekly reports to Business Development, CEO, and Clinical Services Director
Assist facility leaders to identify trends to continue program expansion and reduce barriers to treatment

Requirements
Strong communication skills to effectively engage with patients and staff
