The Clinical Team Manager provides day-to-day supervision and oversight of direct care team members to ensure the achievement of clinical and administrative outcomes. They are responsible for coordinating patient care plans, managing staff assignments, and ensuring regulatory compliance within the home health benefit period.
Candidates must be a Registered Nurse with an associate degree and at least 2 years of home care experience in a Home Health or Hospice environment. A valid RN license, a dependable vehicle, and the ability to lift up to 50 pounds are required.
The Clinical Team Manager (CTM) is a registered nurse who provides day-to-day support to direct care team members (RNs, LPN/LVNs, CNAs, Therapists, etc.) including supervision and oversight of patient’s plan of care, achievement of both clinical and administrative outcomes, and professional and supportive services to clients in a safe, cost-effective manner. The CTM is responsible for the coordination of care during the home health benefit period, staff engagement and communication, safety, and training of staff.
