The Case Manager I provides care planning and connects youth and families with community resources to ensure their educational, mental health, and physical needs are met. They also maintain client records, perform assessments, and advocate for families while facilitating service connections.
Candidates must have a High School Diploma or GED, with a Bachelor's degree in Social Work or a related field required for the Raise the Age Program. A minimum of 2 years of experience in Human Services specializing in youth and family services is required, along with a valid driver's license.
The Case Manager I supports the clinical team by providing care planning, supporting service and community referrals. The Case Manager I supports Aftercare Services by helping to connect youth and family with community resources after discharge from residential care in order to ensure that educational, mental health, physical and psychiatric needs are met. Some Case Manager positions require meeting with and monitoring youth’s successful engagement with their family and community for up to 6 months.

May be required to physically restrain clients weighing between 50 and 300 lbs. and guide them safely to the floor. In some circumstances, restraints can last up to 15 minutes and may require repetition as necessary.
$21.50 Minimum pay rate, $31.00 Maximum pay rate, based on experience
