The Case Manager facilitates patient care from pre-admission through post-discharge by coordinating with the interdisciplinary team to ensure medically appropriate and cost-effective care. They are responsible for conducting utilization management, managing discharge planning, and ensuring compliance with regulatory standards.
Candidates must hold a Bachelor's degree in Nursing and a current New York State Registered Professional Nurse license. A minimum of five years of clinical experience is required, with preference given to those with experience in utilization management or discharge planning.
Facilitates patient's hospitalization from pre-admission through post-discharge. Coordinates with physicians, nurses, social workers and other health team members to expedite medically appropriate, cost-effective care. Assesses, plans, oversees and evaluates the appropriateness of care across the acute care continuum. Applies clinical expertise and medical appropriateness criteria to resource utilization and discharge planning.
