Case Manager For JASA
Shift Schedule: Monday–Friday, 9:00 AM – 5:00 PM Hours Per Week: 35
Under the direction of a Master’s-level supervisor, you will provide critical crisis intervention, protective services, and social casework for older adults who are mentally or physically impaired, abused, or neglected. This role operates on both a voluntary and involuntary basis, requiring a high level of professional judgment and a commitment to elder justice.

📝 What You’ll Be Doing
You will manage a complex caseload, performing everything from emergency triage to long-term protective planning.
Key Responsibilities:
Crisis Triage & Intake: Conduct preliminary screenings and triage for emergency interventions; provide immediate information and referral services to clients and families.
Comprehensive Assessment: Perform in-home and in-office assessments of social and emotional needs using standardized instruments (e.g., PSA, EISEP-PATH).
Case Planning & Implementation: Develop written case plans including individual/family counseling, benefit screening, and arrangement of services (Home Health Aides, Legal Aid, Housing, etc.).
Financial & Legal Protection: Evaluate clients' capacity to manage their affairs; assist in securing financial management or guardianship to prevent elder abuse and financial exploitation.
Field Monitoring: Conduct ongoing in-home visits to monitor risk, evaluate functioning, and revise case plans in real-time.
Emergency Response & On-Call: Participate in a rotating on-call schedule (evenings/weekends/holidays). Respond to "cold weather days" and crisis situations requiring emergency medical or court-ordered actions.
Documentation: Complete required case records, reports, and statistics within mandated timeframes utilizing specialized software.
🎓 What You’ll Need
Education: Graduate of an accredited college or university with a BSW or BA/BS (Psychology or Gerontology preferred).
Technical Skills: Computer literacy and the ability to maintain meticulous records.
Communication: Proficiency in English required; knowledge of Yiddish, Russian, Spanish, or Patois is helpful.
Physical Requirements: Ability to travel to clients’ homes using mass transit and navigate physical barriers (stairs, etc.) in residences.
