The Clinical Coordinator provides clinical support and case management for aging services programs using a Critical Time Intervention model. They are responsible for supervising staff, managing a caseload of 30-40 clients, and overseeing service delivery workflows to support housing retention.
Candidates must hold a NYS Licensed Masters Social Worker (LMSW) credential and have at least 3 years of experience with related populations. Proficiency in Microsoft Office and experience with clinical service models like Housing First and Harm Reduction are required.

Supportive housing - affordable housing paired with services designed to help people maintain their homes for the long-term - is widely recognized as a proven and cost-effective solution to chronic homelessness.
But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.
Reporting to the Assistant Director, Living in Place, the Clinical Coordinator, Living in Place, provides clinical support to Queens based, Aging Services programs at Breaking Ground using a Critical Time Intervention (CTI)-informed model; providing time-limited services focused on housing retention, resource development, and community integration intended to support older adults with aging safely in the community, and provides information and referral services; benefits application and navigation assistance; resource development support; and community integration programming. The case management for Aging Services is typically a 3–6 month engagement and on a referral basis.
The Clinical Coordinator maintains a caseload of 30-40 clients and serves as a member of the Tenant 360 management team.
This position has a 35-hour work week. This is a fully in-person position that will divide time across the Breaking Ground building portfolio. The position may require occasional evening or weekend hours to support tenant events.
