To provide direct medical care to patients of FoundCare as part of the medical provider team, including primary care, chronic disease management, and minor procedures. Unless specifically exempted by the Chief Executive Officer, Physicians are qualified to provide inpatient care, assume on-call after-hours responsibilities, and supervise the health center’s PAs and NPs as appropriate.

Services include pediatric, geriatric, and adult primary care, chronic disease management, behavioral health services, dentistry, laboratory services, women's health services (including screening mammography), and X-rays. We are here to serve your healthcare and dental needs. Our mission is to provide quality healthcare and social services for all individuals and families.
