The Registered Nurse Case Manager provides personalized, one-on-one skilled nursing care to patients in their homes to support recovery and independence. They are responsible for developing individualized care plans, monitoring patient progress, and coordinating with a multidisciplinary team of healthcare professionals.
Candidates must hold a current and unrestricted Registered Nurse license and possess experience in med-surg, ICU, ER, or acute care settings. A valid driver's license, reliable transportation, and current CPR certification are also required.
As a Home Health Registered Nurse, you will:

Med surg, ICU, ER, or acute experience
Current and unrestricted Registered Nurse licensure
Current CPR certification
Experience collaborating with a team of healthcare professionals
Valid driver's license, auto insurance and reliable transportation
