The Case Worker is responsible for implementing and monitoring case management activities to help underserved individuals secure permanent housing, benefits, and healthcare. Key duties include conducting intake assessments, creating case plans, and networking with community partners to stabilize clients' lives.
Candidates must have an Associate's degree or a High School diploma with four years of relevant experience, plus two years of experience in social or human services. A Bachelor's degree and bilingual proficiency in Spanish or Mandarin/Cantonese are preferred.

POSITION OVERVIEW:
Under the supervision of the Program Director, the Case Worker will be responsible for implementing, monitoring, and evaluating case management activities for the program. The Case Worker works directly with individuals dealing with homelessness, health care referrals, processing paperwork, etc. The Case Worker is responsible for assessing the needs of the individuals and their families and helping them secure permanent housing, benefits, and access to other available social service incentives.
