The Field Care Manager assesses and evaluates members' behavioral health and psychosocial needs to develop and implement member-centered care plans. They coordinate services across providers and community resources while conducting field visits to support improved health outcomes.
Candidates must possess a Master's degree in a behavioral health field with active Kentucky licensure (LCSW, LPCC, LMFT) or a Bachelor's degree in Nursing with an active RN license. At least 2 years of case management experience and the ability to travel up to 75% in Eastern Kentucky are required.
The Field Care Manager, Behavioral Health 2 employs evidence-based strategies to manage members’ behavioral, physical, environmental, and psychosocial needs. Responsibilities include identifying barriers, coordinating care, monitoring progress, and adjusting interventions to support outcomes.

Visits occur in homes, community settings, hospitals, shelters, and correctional facilities. These visits are essential to the role.
Work Environment
TB Statement: This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Driving Statement: This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.
