What You'll Do
As a Hospice RN Case Manager, you will
Deliver skilled nursing care to patients in their homes/facilities in accordance with physician orders and the individualized plan of care. Complete comprehensive assessments to identify each patient's physical, emotional, psychosocial, and spiritual needs. Develop, implement, and update patient-centered care plans in collaboration with patients, families, caregivers, and the interdisciplinary team. Educate patients and families about disease progression, symptom management, medications, and available resources to support informed decision-making. Provide expert pain and symptom management to promote comfort and quality of life. Coordinate care across disciplines to ensure seamless communication and continuity of services. Maintain accurate, timely documentation in accordance with agency, state, and federal guidelines. Advocate for patients and families while supporting their goals and wishes throughout the hospice journey.
What You'll Bring
Current Registered Nurse (RN) license in the State of Maine. Minimum of one year of recent clinical nursing experience preferred Current CPR certification. Experience in hospice, home health, palliative care, or community health is preferred. ANA certification in Home Health or Community Health Nursing is encouraged. Excellent assessment, communication, and critical thinking skills. Reliable transportation with valid driver's license and current automobile insurance.
