Responsibilities
The Transitional Coordinator will facilitate the intake and admission process while collaborating with clinical teams to develop and implement person-centered transition plans. They are responsible for conducting risk assessments and providing ongoing support to individuals transitioning into community services.
Requirements
Candidates must hold a Master’s degree in Psychology, Social Work, or Counseling and possess at least five years of experience working with individuals with IDD and forensic involvement. Extensive expertise in developing Individual Program Plans and linking clients to community resources is required.
Collaborate with DDS and the Regional Centers on the Intake and Referral Process.
Facilitate and lead Intake/admission meeting.
Collaborate with the Clinical Coordinator, Consultants, and other members of the IDT in the development and implementation of the IPP and Person Centered Plan and transition plans
Assist in conducting Risk Assessments in collaboration with the Director and Clinical Director
Coordinate with Regional Center Liaison and Service Coordinator on accessing services in the community as per IPP and Transition Plan
Support the person pre transition, during transition and post transition with at a minimum of monthly visits or more frequently as necessary.
Performs all other duties, as assigned.
Pay Rate:$87,560.30 annually
Minimum Qualification Requirements including education, experience, licensure/certification, etc. and essential physical capabilities (e.g. lifting, assisting lifting, standing, etc.)
A Master’s Degree in Psychology, Social Work, Counseling and five years of experience in the past 7 years working with people with IDD with behavioral/mental health concerns with criminal and/or forensic involvement.
Extensive experience in the development and implementation of Individual Program Plans and person-centered plans for people with IDD with criminal and forensic involvement.
Extensive experience in linking people with IDD with complex behavioral/mental health and forensic needs to community supports and resources.
Credentialed Alcoholism and Substance Abuse Counselor (CASAC) credential
