Position Summary

The Access Coordinator is responsible for ensuring seamless access to home health services for patients and clients. This role coordinates and optimizes the intake process, manages referrals, and maintains communication with healthcare providers, patients, and families to facilitate efficient service delivery and enhance the overall patient experience.

Key Responsibilities

Coordinate the intake process for new patients, ensuring all required documentation and information is collected, verified, and processed accurately.
Manage incoming referrals from healthcare providers, ensuring timely review, follow-up, and communication throughout the referral process.
Serve as the primary point of contact for patients, families, referral sources, and healthcare providers, responding to questions and facilitating access to services.
Collaborate with clinical staff to ensure patient care plans are initiated promptly and align with individual patient needs and preferences.
Maintain a thorough understanding of Andwell's home health services, eligibility requirements, policies, and procedures to provide accurate information and guidance.
Monitor referral and access metrics, identify trends, and support continuous process improvement initiatives to enhance efficiency and patient satisfaction.
Assist in developing and implementing access-related policies and procedures while ensuring compliance with organizational standards and regulatory requirements.
Maintain accurate and timely documentation within the electronic health record and other applicable systems.
Perform additional duties as assigned to support the organization's mission and operational needs.

Qualifications

Associate's degree in healthcare administration, business, or a related field preferred; equivalent combination of education and experience will be considered.
Previous experience in healthcare scheduling, patient access, intake, referrals, customer service, or care coordination preferred.
Knowledge of home health services and healthcare regulations is a plus.
Excellent communication and interpersonal skills with the ability to build positive relationships with patients, families, providers, and colleagues.
Strong organizational, time management, and multitasking abilities.
High attention to detail with the ability to maintain accuracy in a fast-paced environment.
Proficiency with electronic health records (EHR) and Microsoft Office applications.
Commitment to delivering exceptional customer service and supporting high-quality patient care.

QualificationsRequired Education
