HUMAN SERVICES COUNSELOR III 

Your Specific Responsibilities: 

· Establish a trusting relationship with culturally diverse families by initiating and maintaining regular client contact. 

· Provide Case Management to pregnant women and infants referred to the Healthy Start Program. 

· Provide face to face contact with clients either in the OB provider’s office or in the client’s residence. 

· Provide Parenting Education one on one with clients. 

· Provide Breastfeeding Education and Tobacco Cessation Education to individual clients. 

· Document all client contact information in the web-based computer application known as the Well Family System 

· Provide referrals to community resources to supplement Healthy Start services. 

Required Knowledge, Skills, and Abilities: 

· Fluency in English and Spanish 

· Experience working with pregnant women and children. 

· Problem solving skills related to advancing client self-sufficiency. 

· Ability to work well with others as a team. 

· Computer skills: competency and experience. 

· Experience in interviewing and assessing to determine the needs of the client. 

· Knowledge of community resources. 

Qualifications: 

Minimum- 

· Valid Florida Driver's License 

· Minimum of four-year college degree in one of the following areas; Social Sciences; a health-related field such as Nursing, Health Education, Health Planning, or Health Care Administration; or Social Work as required by FL Healthy Start Standards and Guidelines (Chapter 6-2) 

· At least 1 year experience working with high-risk pregnant women and/or families 

· Must have reliable and dependable transportation every day for home visitation purposes 

Preferred- 

· 1 to 3 years of experience working with families 

· 1 to 3 years home visiting experience 

· 1 to 3 years case management experience  

· Bilingual with fluency in English and Spanish
