Qualifications:
Education: Bachelors preferred.

General Description
The Clinical Supervisor is responsible to the Residential Services Director for the daily operation of the community residence in Bath and scattered sites in Bath and Hornell. Additionally, the Clinical Supervisor is the designated health care coordinator of the programs. The Clinical Supervisor is the immediate supervisor of all staff within the community residence and scattered sites.
Essential Duties and Responsibilities
Oversees the delivery of comprehensive services with a therapeutic environment at the community residence and SLP.
Ensures the admission and orientation paperwork meets OASAS standards and completes the admission indicator and level of care assessments.
Ensures all residents have a medical history recorded and a physical exam performed within 45 days of admission.
Ensures the following: comprehensive service plans are prepared within 30 days of the development of the initial service plan, service plans are reviewed at least every 30 days thereafter and signed as required by the supervisor.
Ensures the discharge plans provide for the specific needs of a resident and prepared in collaboration with the resident.
Ensures all discharge plans are completed and approved prior to resident discharge.
Ensures progress notes are prepared at least once per week and reflect the goals and objectives of the service plan.
Ensures all records are locked and medications are stored and monitored according to program policy.
Ensures all residents receive comprehensive case management services and complete access to community services for legal, mental health, social services, vocational assessments, and misc. needs.
Ensures LOCADTRs are completed within 24 hours of admissions and prior to all discharges.
Receives phone referrals, sets up screening appointments and writes notes on assessment interviews which are sent to Program Director for approval.
Works closely with mental health professionals and medical staff in regards to clients’ medical/psychological issues, relating to appropriateness for admission.
Submits reports concerning the number of clients seen and disposition.
Submits census reports indicating the number of clients on the waiting list, and the number of clients in the program, discharges, and new admissions.
Maintains relationship through ongoing outreach with all potential referral sources.
Ensures the provision of education, risk reduction, counseling and referral services to all residents regarding HIV (including pre-and post-exposure prophylaxis), tuberculosis, viral hepatitis, sexually transmitted infections and other transmissible infection consistent with 820.6(d) and LSB 2019-03 (SAPT) Block Grant Requirements.
Administrative Responsibilities:
Ensures the adherence of confidentiality regulations.
Ensures the community residence and scattered sites meet the OASAS facility requirements for safety and cleanliness.
Ensures fire drills are conducted on a monthly basis and at a variety of times.
Ensures utilization reports are completed and turned into the Residential Services Director at the beginning of every month.
Ensures the timely submission of all applicable OASAS data reports relative to the provision of services at the beginning of every month.
Meets with the Residential Services Director for supervision, attends agency meetings as assigned by Program Director, and facilitates case reviews.
Supervisory Responsibilities:
Ensures staff will receive orientation and on the job training.
Ensures staff will have scheduled supervision and periodic performance evaluations.
Ensures staff will receive training in the following areas: chemical dependence, individual group and family counseling, therapies and other activities supportive of recovery, medication assisted treatment, motivational interviewing, trauma informed care, communicable diseases, infection control, supervision and quality control.
Ensures the staff provide quality of care to those we serve.
Completes other duties as assigned
