The Physical Therapist will plan and administer personalized rehabilitation programs for patients within their homes to improve functional independence. They are also responsible for documenting patient progress and coordinating care with physicians and the broader healthcare team.
Candidates must hold a current and unrestricted Physical Therapy license and a valid CPR certification. Experience collaborating with a team of healthcare professionals is required, with at least one year of experience preferred.
