About the Role
As a Housing Case Manager, you will play an important role in helping women (trans-inclusive) who reside in the YWCA NENY housing program navigate their path toward stability and independence.Our residents come from a wide range of backgrounds and experiences, including survivors of domestic violence, individuals experiencing homelessness or extreme poverty, people living with mental health challenges, individuals in recovery, people returning to the community following incarceration, and others who have experienced significant barriers to stable housing.
This is meaningful, relationship-centered work. You will build trust with residents, help them identify goals, connect them with resources, advocate alongside them, and celebrate progress, both big and small.

What You’ll Do
Provide case management and support: Build trusting relationships with residents while providing housing information, emotional support, referrals, guidance, advocacy, and assistance in identifying goals and navigating barriers to stability and independence.
Support housing access and intakes: In partnership with the Housing Director, conduct housing intakes, manage the housing waitlist, and assist with Schenectady Coordinated Entry System intakes.
Connect residents to resources: Maintain current knowledge of local social services, housing resources, tenant rights and responsibilities, and YWCA rental procedures to help residents access appropriate services and opportunities.
Maintain accurate records and data: Complete case management notes, resident files, daily tracking, statistical data, and reports related to homeless services.
Promote a safe and supportive community: Work with the Director of Housing, case management team, and maintenance staff to maintain a safe living environment; conduct room checks with another case manager or the Director and assist with room turnover as needed.
Build community and well-being: Facilitate monthly group activities that promote resident connection, engagement, and well-being.
Assist residents with appointments and services: Provide transportation to appointments using the agency van when needed.
Collaborate and participate: Attend staff and case management meetings and required trainings, while contributing to ongoing team and program effectiveness.
Support program operations: Provide occasional front desk coverage and perform other duties as assigned to support the program and agency.
