Detailed Overview
Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families: The Intensive Support & Stabilization Team (ISST) provides urgent and short term 24 hour home support services to clients across Fraser Health and the Coordinator is responsible for the daily operation of services within an assigned geographic area. Coordinates designated staff including overseeing the scheduling of home support services and ensuring adequate staffing levels, including compliance with collective agreement provisions, and providing performance feedback and conducting related appraisals. For the assigned geographical area, coordinates human resource related events such as the hiring of ISST staff, contributing to and delivering standardized training and orientation programs to staff, and providing technical and procedural guidance to the team. Receives and organizes initial investigation of non-clinical related complaints and grievances and develops statistical and various operational reports that help inform quality improvement recommendations.
Responsibilities
Provides administrative coordination and oversight to the team within the assigned geographical area, in collaboration with the Team Leader and Manager, Clinical Operations and Program Services.
Oversees and coordinates designated staff within the assigned geographical area, including the scheduling of ISST services, clerical support, performance feedback and review, discipline, and selecting staff as required.
Coordinates and works to resolve human resource events such as non-clinical complaints related to ISST services and staff grievances; collaborates and consults with appropriate Employee Experience teams and works in conjunction with the Manager, Clinical Operations and Program Services.
Contributes to the development of and provides standardized training, orientation, guidance and implementation of effective processes and guidelines related to ISST operations.
Coordinates the implementation of quality improvement initiatives including reviewing and recommending process changes for workflow, policy and process, staffing levels, and use of technology. Assesses impact of changes on clients and staff. In conjunction with the Manager, Clinical Operations, implements approved changes and guides staff through changes.
Develops, compiles, coordinates, analyzes and monitors various operational reports related to ISST. Provides quality-related and other statistics and prepares related reports on a continual and ad hoc basis that help inform quality improvement recommendations.
Conducts audits related to compliance of standards and quality improvement of client care. Analyzes and evaluates audit findings and develops recommendations to the Manager, Clinical Operations for continuous quality improvement.
Participates in various meetings related to ISST. Assists and/or undertakes various projects as assigned by Manager, Clinical Operations and Program Services.
Qualifications
Bachelor's Degree in a health related discipline plus five (5) years of recent related experience working in a home support or long term care setting, or an equivalent combination of education, training and experience.
