At Father Joe’s Villages, Street Health meets people where they are - in the places most systems don’t reach. Our Street Health team provides outreach, engagement, and integrated healthcare support for individuals experiencing homelessness across San Diego County, helping people improve health outcomes and take meaningful steps toward stability and housing.
As a Street Health Outreach Medical Assistant, you will play a unique hybrid role that combines, field-based outreach, care coordination support, and clinical medical assistant duties. You will meet patients in the community, build trust over time, and connect individuals to the healthcare system while supporting ongoing care in both field and clinic settings.
What Success Looks Like
Success in this role means building trusted relationships with individuals who are often disconnected from care and helping them stay engaged in services over time.
You will work in dynamic environments - including streets, encampments, shelters, and clinics - supporting individuals with complex medical, behavioral health, and social needs. Your ability to combine clinical skills, outreach, and care coordination will directly improve patient engagement, continuity of care, and overall health outcomes. This is a full-time position involving field-based outreach and clinic-based work. Duties are performed across streets, encampments, shelters, and clinical environments. Schedules are typically Monday through Friday; hours may vary based on outreach schedules, patient needs, and program priorities.
Compensation & Benefits
The anticipated starting pay range for this role is $24.00 to $27.61 per hour. Final placement and starting rate will be determined based on the candidate's experience, qualifications, certification status, and internal equity considerations.
