About Health Independence Program
Austin Health's Health Independence Program (HIP) sits within the Continuing Care Directorate and is funded by the Victorian Department of Health. HIP delivers multidisciplinary services that support people with health conditions and other complex needs to maximise their functional ability, build self-management skills, and maintain independence within their homes and communities.
The program includes a range of services designed to support a smooth transition from hospital to home. These services can also help reduce unnecessary hospital presentations and admissions, with some programs accessible directly from the community.
HIP brings together a number of interconnected services that provide integrated, coordinated, and client -centred care. These services work collaboratively to address each client's individual needs, goals, and circumstances, promoting improved health outcomes and quality of life while supporting people to remain safely and independently in the community.
About the role
Older Persons Complex Care (OPCC) is a rapid-response service for non-admitted clients that combines home visits and telehealth to provide comprehensive assessment, specialist geriatric care, and short-term clinical monitoring for older adults with complex health needs. OPCC aims to support clients in their own homes, reduce avoidable hospital admissions, and facilitate timely access to specialist care.
Our aim is:
To support timely discharge from hospital and transition to GP and /or other services for ongoing long-term care.
Reduce unplanned hospital admission through the Emergency Department.
To complete a comprehensive client focused assessment, and provide support and education to clients with complex medical issues and their family in their own homes.
As one of the two the Nurse Practitioners in the service, you will work collaboratively with our clients, providing your expert clinical lens to support sustainable healthcare in the home environment.
The Nurse Practitioners in the service works collaboratively with our Registered Nurse, Fellow in Geriatric Medicine and a Geriatrician in OPCC to support the ongoing needs of our clients and the service.
