The Client Care Coordinator manages the delivery of mental health services and coordination of care for homeless individuals at the House East location. Candidates must be a Licensed Master Social Worker (LMSW) in New York or obtain licensure within three months of hire. Preferred qualifications include at least one year of experience in social or homeless services and proficiency in Spanish or French.
PRIMARY FUNCTION/PURPOSE:
The Client Care Coordinator is responsible for the overall delivery and coordination of mental health and related services for homeless individuals. The Client Care Coordinator will enhance existing services to include a thorough assessment of clients’ long-term goals at intake and collaborate with the case management staff to set achievable goals while establishing linkages within the community to ensure their successful progress towards independent living. This position will work with individuals at our House East location in New York, NY.
