Coordinate essential services to maximize recovery and independence for individuals with mental illness. Perform psychosocial assessments and facilitate the implementation of individualized service plans within the community.
A bachelor's degree in a human services field is required, with professional licensure (e.g., LMSW, LMFT) preferred. Candidates must possess a valid Michigan Driver's License and preferably have prior experience in mental health case management.
This position does require working in the community and performing home visits.
The Case Manager primary responsibilities include: Coordinate all services necessary to maximize consumer recovery and independence for individuals with mental illness. In a residential or clinical setting, provide clinical and program consultation to direct care staff. Ensure case management compliance with agency standards and the practice model. Assist consumer to improve quality of life and maintain the highest possible ability to recover and function within the community. Implement strategies for obtaining services and supports that are goal-oriented and individualized.
