The RN Case Manager assesses the physical and emotional needs of hospice patients to create and implement personalized care plans. They provide direct nursing care, manage pain and symptoms, and collaborate with an interdisciplinary team to support patients and their families.
Candidates must be a graduate of an accredited nursing school with a current state license and at least one year of nursing experience. A valid driver's license, insured vehicle, and preference for hospice experience are also required.
