The Home Health Admit RN performs initial patient assessments, medication reconciliation, and creates personalized care plans in a home setting. They also provide wound care, administer infusion therapies, and manage clinical documentation while coordinating care with the interdisciplinary team.
Candidates must be a licensed Registered Nurse in the State of Arizona with a valid driver's license and reliable transportation. At least one year of home health experience, along with wound care or infusion therapy experience, is preferred.

This is a part time/PRN position working every Saturday and Sunday!
*This position will provide service in the West Valley (including Sun City, Sun City West, El Mirage, Surprise, Litchfield Park and Goodyear).
