The Home Health Admit RN performs initial patient assessments, medication reconciliation, and home safety evaluations to create personalized care plans. They also provide direct clinical care including wound treatments and infusion therapy while coordinating with the interdisciplinary team.
Candidates must be a licensed Registered Nurse in the State of Arizona with current CPR certification. A valid driver's license and reliable transportation are required, with preference given to those with home health, wound care, or infusion experience.
