The Social Work Lead provides clinical supervision and oversight to social work staff while managing a full caseload of participants. They participate in the Interdisciplinary Team to develop treatment plans, conduct assessments, and ensure compliance with regulatory standards.
Candidates must hold a Master's degree in Social Work and a valid LMSW license in the State of Michigan. The role requires at least three years of clinical social work experience, preferably in geriatrics, and one year of leadership or supervisory experience.
The Social Worker Lead (Licensed Master’s Social Worker) (LMSW) works independently to assess the needs of, coordinate, and deliver social services for participants and families/caregivers. The LMSW is a member of the Interdisciplinary Team (IDT) and participates in the assessment, intervention, management, and review of social service needs. The LMSW also assists the IDT in determining eligibility and care needs. The LMSW provides care through a variety of modalities, including but not limited to, bio-psychosocial assessment, interventions that include individual and group counseling utilizing motivational interviewing, and cognitive behavioral techniques, psychoeducation, consultation, and care planning/case management. Additionally, the Lead Social Worker is responsible for daily clinical management, supervision, and oversight of social workers.

Our interdisciplinary team approach is a unique model of care that allows our staff to personally get to know participants and their caregivers. Our team meets on a daily basis to proactively communicate and coordinate holistic care to prevent avoidable emergency visits, hospitalizations and nursing home placements, thus providing higher quality care.
