The Case Manager coordinates safe and timely discharge planning for inpatient psychiatric patients by collaborating with families and community providers. They facilitate continuity of care through referral management, transportation arrangements, and accurate documentation of all transition activities.
An Associate degree is required, though a Bachelor's degree in Social Work or a related field is preferred. Candidates should ideally have one to two years of experience in behavioral health or healthcare case management.
Position Summary: The Case Manager is responsible for coordinating safe, timely, and clinically appropriate discharge planning for patients receiving inpatient psychiatric treatment. Working collaboratively with patients, families, treatment teams, community providers, and external agencies, the Case Manager facilitates continuity of care by identifying discharge needs and barriers, coordinating referrals and aftercare services, arranging transportation, and ensuring all discharge planning activities are documented accurately and completed within established timeframes. This role requires proactive case coordination from admission through discharge to support successful patient transitions and positive treatment outcomes.
Education: Associate Degree required; Preferred - Bachelor's degree in Social Work, Psychology, Human Services, or a related field required.
Experience: One to two years of case management, discharge planning, behavioral health, or healthcare experience preferred. Must demonstrate effective care of coordination, referral management, documentation, and collaboration with multidisciplinary teams and community providers.
