Position Summary
Intensive Case Management Services (ICMS) provide comprehensive, community-based services to individuals experiencing or exiting homelessness who have complex medical, behavioral health, and social service needs. Using a Housing First and Harm Reduction approach, Case Managers help residents living in Permanent Supportive Housing (PSH) achieve housing stability, improve health outcomes, and increase independence.
This position is ideal for individuals who thrive in a fast-paced, relationship-focused environment and are passionate about serving vulnerable populations.
Essential Duties & Responsibilities
Provide intensive, client-centered case management services to residents living in Permanent Supportive Housing (PSH).
Develop trusting relationships that promote housing stability, recovery, independence, and improved physical and behavioral health.
Conduct comprehensive psychosocial assessments and develop individualized service plans, reviewing and updating plans at least quarterly.
Coordinate referrals and linkage to primary care, behavioral health, substance use treatment, benefits, employment, legal services, and other community resources.
Conduct home visits, wellness checks, and community-based services where clients live, and accompany clients to appointments as needed to support engagement and continuity of care.
Respond to behavioral health, housing, and other client crises using trauma-informed crisis intervention, de-escalation, and coordination with emergency responders, hospitals, and community providers when appropriate.
Maintain timely and accurate documentation, assessments, service plans, and progress notes in CHAMP, HMIS, APRICOT, and other required systems.
Manage a caseload in accordance with County ICMS staffing requirements. Caseload size varies based on client acuity and program needs. As client acuity decreases, Case Managers may assume additional clients or provide coverage at other program locations to maintain County staffing ratios.
Collaborate with multidisciplinary teams, property management, healthcare providers, and community partners to support resident success and housing retention.
Practice Housing First and Harm Reduction principles while respecting client choice, dignity, and self-determination.
Participate in team meetings, trainings, quality improvement activities, and other duties as assigned.
What to Expect
This is not a traditional office-based case management position. While Case Managers are assigned to a primary office location, some of the work requires home visits and travel to community location. The position requires flexibility, critical thinking, and the ability to respond to changing priorities, urgent client needs, and crisis situations while maintaining professionalism and strong clinical judgment.
Qualifications
Minimum Requirements
Bachelor's degree in Social Work, Psychology, Human Services, or a related field and at least one year of experience working with individuals experiencing homelessness; or
Two years of experience providing direct mental health services or intensive case management.
Preferred Qualifications
Experience working with individuals experiencing homelessness, serious mental illness, substance use disorders, and chronic health conditions.
Knowledge of Housing First, Harm Reduction, trauma-informed care, and community resources.
Strong communication, organization, documentation, and time management skills.
Experience with electronic documentation systems (HMIS, CHAMP, APRICOT, or similar).
Ability to work collaboratively while managing competing priorities in a community-based environment.
Ability to work occasional evenings or weekends as program needs require.

Century Villages at Cabrillo (CVC) is a residential community established to break the cycle of homelessness. A former 26 acre US Naval housing site, CVC has become a nationwide public-private partnership model to benefit those in need. A 501(c)3 nonprofit organization, CVC was established in 1997 when the US Department of Defense transferred the land to CVC under the McKinney Act for the purpose of providing transitional and permanent housing to the homeless and those at risk of becoming homeless.

These include veteran and non-veteran individuals, families, youth and children who are housed within CVC’s robust continuum of supportive housing, ranging from shelter, to transitional housing, to permanent housing. CVC’s mission is to develop, manage, and serve as steward of this 27 acre supportive housing campus so that independent service providers can successfully help individuals and families overcome homelessness.
