Administer personalized physical therapy treatment plans to patients within their homes to improve strength, mobility, and independence. Maintain accurate clinical documentation and coordinate with the Physical Therapist and Branch Director regarding patient progress and equipment needs.
Requires a current state license to practice Physical Therapy, a valid driver's license with auto insurance, and CPR certification. Candidates should have comprehensive knowledge of geriatric population changes and fluency in English.
As a Home Health Physical Therapist Assistant, you will:
Required Qualifications:
Current state license to practice Physical Therapy
Valid driver's license, auto insurance and reliable transportation
Current CPR certification
Experience collaborating with a team of healthcare professionals
Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population
Read, write, and speak fluent English
Preferred Qualifications:
One year of experience as a Physical Therapy Assistant
Pay Per Visit/Unit Rate
$41.00 - $57.00

TB Statement:
This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Driving Statement:
This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance.
