Employment Duration: 3 months (w/ potential for extension)
The Licensed Vocational Nurse, under the supervision of the Administrative Nurse, guided by the policies and procedures of Ambulatory Services and the Ambulatory Services Quality Nursing Division, and in collaboration with the physician staff, has the clinical responsibility for developing and maintaining a high standard of patient care. The LVN is responsible for maintaining a safe environment for patients and staff and supporting daily operations. All components of nursing care are individualized to meet the patient’s physiological and psychosocial developmental level and needs. The LVN consistently projects a professional and courteous demeanor and complies with Medical Center House and Telephone Standards.
Key Responsibilities:
Assess patients’ needs and responses to medical regimens and treatments through observation of symptoms, emotional state, and cultural and socioeconomic backgrounds.
Recognize and report physical and emotional patient conditions to appropriate members of the health care team, including urgent changes requiring immediate provider attention.
Collaborate with the health care team to develop patient care goals and treatment plans.
Identify appropriate internal and external resources to support patient care planning.
Provide individualized nursing care that meets patients’ developmental and clinical needs.
Prepare exam rooms and perform the patient intake process while ensuring patient privacy.
Prepare and instruct patients regarding flu vaccine procedures and treatments.
Assist with diagnostic and therapeutic procedures and treatments.
Safely use and maintain clinical equipment and appliances.
Evaluate and document patient responses to nursing care, procedures, and treatments.
Administer medications and document administration while maintaining current pharmacology knowledge and competency.
Respond appropriately to medical emergencies, including assisting with CPR and emergency procedures.
Follow all Ambulatory Services and Medical Center policies related to infection control, safety, administrative, and clinical practice.
Complete annual competency requirements as specified by the practice.
Assist patients with limited mobility during transfers.
Organize clinical activities to maintain efficient patient flow.
Communicate effectively with patients, families, and members of the health care team to support continuity of care.
Maintain accurate, current, legible, and properly signed patient documentation.
Obtain, record, and communicate patient information clearly and concisely.
Demonstrate professional telephone etiquette and comply with UCSF Medical Center Telephone Standards.
Foster positive relationships with patients, families, visitors, and staff while demonstrating cultural sensitivity.
Identify patient educational needs and provide information to support informed decision-making.
Reinforce patient education using language patients and families can understand and verify comprehension.
Utilize and maintain educational resources and participate in the review of educational materials.
Participate in performance improvement activities, data collection, and initiatives that improve departmental and clinic performance.
Report and follow up on environmental and safety concerns.
Support cost-effective practices and appropriate use of departmental resources.
Attend applicable classes, conferences, workshops, staff meetings, and clinical team meetings.
Maintain compliance with work-related policies, including license renewal, CPR certification, required health screenings, and identification requirements.
Contribute to the orientation and development of other members of the health care team.
Follow departmental scheduling guidelines and complete assigned work in a timely manner.
Perform other duties as required.
