The Community Health Navigator conducts outreach to enroll members, assesses their needs, and builds personalized care plans to address social determinants of health. They manage a caseload of 80-120 members, coordinating referrals and providing ongoing support to help members overcome barriers to care.
Candidates should have at least 1 year of experience in social work, human services, or a related field, or a bachelor's degree in a relevant discipline. Strong communication, tech literacy, and the ability to work independently in a fast-paced, remote environment are essential.
