The Social Worker performs psychosocial assessments and interventions for hospice patients and their families as part of an interdisciplinary team. Responsibilities include care planning, grief counseling, patient advocacy, and coordinating community resources.
Requires a Master of Social Work degree from a CSWE-accredited institution and licensure or eligibility. Candidates should have two years of experience in patient, home, or acute care settings, preferably within hospice.
The Social Worker is responsible for performing assessments, psychosocial services and interventions with the patient and family within the scope of practice as a member of the interdisciplinary team. This position requires the exercise of discretion and judgment to evaluate patients and families to determine needs and develop care strategies. The Social Worker performs duties that include assessment, intervention, anticipatory grief counseling, care planning, documentation, collaborative/communication and the efficient use of various resources to hospice patients and families.
