Schedule: Full-time Monday–Friday
Location: Field Based (Baytown and surrounding areas)
Compensation (Productivity Points):$75.00 per point
Who We Are:
Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. Our mission is to dramatically improve patient access to home health care.
The PT role is a core part of that mission: you restore function, keep patients safe at home, and drive real outcomes.
We also build technology to support field clinicians. Our charting product reduces documentation burden, and our scheduling product will make routing and visit coordination easier — protecting your time.
What you will do:
Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits.
Complete comprehensive in-home assessments and deliver skilled PT visits.
Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.
Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.
Manage treatment and outcomes directly, including PTA supervisionwhere appropriate.
Coach patients and caregivers with practical home programs that actually get done.
Document promptly and thoroughly

Requirements
Must-haves
Active Texas PT license (in good standing)
Strong clinical judgment and comfort working independently in the field
Comfortable escalating changes in condition to RN/MD and documenting rationale clearly
Strong communication and documentation
Reliable follow-through on orders, frequencies, and patient goals
Current driver’s license and comfort with daily travel across assigned territory
Able to carry/transport basic therapy equipment as needed
