The Admissions RN evaluates prospective patients for hospice eligibility and coordinates the transition into hospice services. They work closely with the interdisciplinary team to initiate individualized plans of care and ensure a compassionate admission experience.
Candidates must hold a current, unrestricted Registered Nurse license and have at least one year of hospice nursing experience. A valid driver's license, reliable transportation, and automobile insurance are also required.

This is an excellent opportunity for a hospice RN who values meaningful patient interaction, professional autonomy, supportive leadership, and a long-term future with a growing post-acute care provider.
The Admissions RN will evaluate patients for hospice eligibility, clearly explain hospice services and benefits, complete the admission process, and initiate an individualized plan of care that creates a seamless transition into hospice services.
The position works closely with clinical leadership, physicians, referral sources, and the interdisciplinary hospice team to ensure every patient receives a timely, compassionate, and well-coordinated admission experience.
This position reports to the Clinical Manager, with additional leadership support from the Chief Operating Officer.
